Looking to get started with services at CI?

We offer free 30 minute consultations to families, so you have the opportunity to ask questions and meet with an Intake Coordinator to discuss our services. After the consult, the first step to getting set up with services is completing our intake paperwork. With your permission on the  INTAKE PAPERWORK we will call on your behalf to request a referral  from your physician’s office and investigate any insurance coverage options that you have access to.
