What insurance companies does CI work with?
We are proud of outstanding and valued relationships with the following insurance companies:
Aetna, The Alliance, Auxiant, ChampVA, GHC, Dean (SSM), Health EOS, Humana, Tricare, Quartz,
UMR, BCBS, Security Health Plan, Aspirus Health Plan, and WPS.
CI is a trusted and contracted provider for most local insurance carriers.
If you do not see your insurance provider on our list, please connect with your employer or insurance carrier.
*CI is not a Medicaid, Badgercare, Forward Health, or Katie Beckett provider.
However, our services can be accessed through a number of county funding programs based on eligibility.

How do I start the insurance process?
Once your intake paperwork is completed, we will help you by contacting your insurance to verify coverage and benefits on your behalf. The CI team will submit any necessary insurance authorizations.
When will I receive a bill?
Our billing cycle is monthly with billing statements being mailed out around the 15th of each month, for therapy services from the previous month.
Who do I contact about a change in my insurance information?
Great question! We will need to know about any change in insurance or address, throughout the year, as soon as possible. You can contact our billing department directly with this information. They will need new paperwork filled out, along with a copy of your new insurance card.
What are my payment options?
There are several ways to pay your bill. You can pay your bill conveniently online, right here, any hour of the day! You can also call in a payment over the phone, 608-497-3198 or you can mail your payment to CI Pediatric Therapy Centers 2935 S Fish Hatchery Rd #140 Madison, WI 53711

County Funded Resources and Programs
The Children's Long-Term Support Waiver (CLTS)
To apply for the CLTS Waiver: Call your county agency
To qualify for CLTS services: The Children’s Long-Term Support (CLTS) Waiver Program is a Home and Community-Based Service (HCBS) Waiver that provides Medicaid funding for children under age 22 who have substantial limitations in their daily activities and need support to remain in their home or community. Eligible children include those with developmental disabilities, severe emotional disturbances, and physical disabilities. Funding can be used to support a range of different services based on an assessment of the needs of the child and his or her family.
Family Support and Resource Center (FSRC)

To qualify for Family Support: Families are eligible for the Family Support Program if they have a child under 21 years of age, who has a significant disability and is living at home in Dane County or returning from an out-of-home placement. Family Support includes a variety of home and community-based supports designed to enhance the quality of life for children with significant disabilities and to keep families together. Based on the belief that each family is unique and that parents are in the best position to know the specific needs of their children and their families, family support services are family-centered and flexible, with parents choosing the assistance they receive.
Include, Respect, I Self-Direct (IRIS)
To apply for IRIS: contact your local Aging and Disability Resource Center
To qualify for IRIS services: IRIS (Include, Respect, I Self-Direct) is a self-directed program for Wisconsin’s adults with disabilities. You have to be eligible for Medicaid to be in the IRIS program and over age 18. IRIS is built on the principles of self-determination and self-direction and can be used for a variety of services.

No Surprises Act
You are protected by the No Surprises Act from surprise billing, also called “balance billing”. Under this law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
